•  Slate:  
• Name: The first and last names of the actor 
• Age: If the performer is younger than 18,  see details at the end of the page for submission guidelines.
• Height: The height of the actor 
• Location: The present state of the actor 
• Representation: The official representatives or agents of the actor 
• Character: The part for which the performer is trying out 
• Union affiliation: The actor's union affiliation, such as SAG-AFTRA

• When self-recording, you may want to think about the following for the audition:

1. Get your lines committed to memory.  DOWNLOAD SIDES (ABOVE)
2. Find a reader to help you. 
3. In the event that you do not have a blank wall, select a neutral background to record. 
4. Reduce background noise and take a position that keeps you in the center and visible at all times. 
5. You have till the deadline to turn in your self-tape.

• A recent headshot that accurately depicts the child's/ teenager's current look. 
• A brief resume mentioning the child or teen's training, seminars, and performance credentials should be included, even if they have little to no acting experience.

• A short monologue or clip showcasing the child/teenager's acting abilities.
